Senior Clinical Support Worker Community Vehicle

We have an exciting opportunity for a Senior Clinical Support Worker to join our Community Liver Health Checks team, working on our mobile clinic vehicle offering liver health checks to the public.


The successful applicant will be based at St James Hospital but will work on our mobile clinic vehicle that covers clinics and events in the community.


The successful applicant will have experience in phlebotomy; Fibroscan training will be provided, which is the main focus of this post, along with education about hepatitis C and B. The team is supported clinically by the nurse team leader.


This post is full-time, but part-time hours would be considered for suitable candidates. The service operates from Monday to Friday, 8 am to 5 pm.

Main duties of the job

The Senior Clinical Support Worker will work as part of the Community Liver Health Checks outreach, predominantly in the community on either the outreach vehicle or community locations in the ODN area.


The postholder will undertake fibroscans and take bloods as required.


Support the viral hepatitis team with community clinics and events.


Work closely with other services such as the Hepatitis C Trust.


Contribute to the multi-disciplinary team.


Be an ambassador for the ODN and treatment services.

Job responsibilities

Job Purpose/Summary


The jobholder will be actively involved in providing direct and indirect compassionate care for patients, supporting the multidisciplinary team.


The jobholder will undertake a range of delegated clinical patient care duties and work under supervision by a registered practitioner.


Assist in planning, implementing, and evaluating care as part of the multiprofessional team.


Maintain high standards of patient care, act on own initiative, and contribute to an effective environment for care.


Lead a team of clinical support workers and apprentice clinical support workers.


Practice in accordance with Trust policies, guidelines, standards, and statutory requirements, operating within defined competency boundaries.


Work within award, clinical department, or community settings as part of a multidisciplinary team.


Principal duties & areas of responsibility

Communication and Working Relationships



  • Establish and maintain effective communication with patients, families, carers, and patient groups.

  • Develop and maintain appropriate relationships with the multidisciplinary team.

  • Assist in welcoming and receiving patients into the clinical area.

  • Demonstrate various communication skills, including with patients with communication needs or difficulties.

  • Communicate effectively with health, social care providers, voluntary sectors, and emergency services.

  • Maintain clear, concise, and legible documentation adhering to standards and policies.

  • Ensure confidentiality and data security in line with Trust policies.

  • Participate in audits and escalate concerns regarding patient needs or clinical changes.

  • Use interpreters as appropriate to support patient communication.

  • Manage phone inquiries effectively.


Clinical Responsibilities



  • Provide high-standard compassionate nursing care, recognizing and meeting patients' specific needs.

  • Undertake clinical activities relevant to the post following training and assessment.

  • Assist in planning, implementing, and evaluating patient care programs.

  • Record interventions and care provided, with countersignatures from registered practitioners where necessary.

  • Assist in fluid balance recording, patient hygiene, safe moving and handling, and other clinical tasks.

  • Perform clinical observations, recognize abnormalities, and report changes.

  • Use early warning scores to detect patient deterioration.

  • Prepare and undertake clinical interventions based on evidence-based practice.

  • Assist in specimen handling and disposal of clinical waste.

  • Involve relatives and carers in patient care where appropriate.

  • Support psychological and spiritual needs using diversion and comfort measures.

  • Act as a chaperone and assist with last offices for deceased patients.

  • Organize and prioritize tasks, and work across wards or departments as required.

  • Safeguard children and vulnerable adults, adhering to safeguarding policies.

Comply with infection control policies, practicing universal precautions and hand hygiene.

Maintain and improve service quality, follow safety policies, and report incidents.

Person Specification
Qualifications

  • NVQ 3/Diploma in Health Level 3 or equivalent.

  • Care Certificate or attain within 3 months.

  • Relevant experience and commitment to ongoing training.

  • Maths and English Level 2 (GCSE 4-9/A*-C).


Additional Requirements

  • Meet Occupational Health requirements, including blood-borne virus clearance.

  • Ability to travel between Trust premises and patient homes.


Experience

  • Healthcare setting experience delivering personal and clinical care.

  • Planning, organizational, and IT skills.
