Job Summary
We are seeking an experienced Practice Nurse to join our multidisciplinary nursing team, which includes GPs, Clinical Pharmacists, Practice Nurses, Health Care Assistants, and Physiotherapists
Main duties of the job
Proficiency in chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care including compression, and other general treatment room duties
Support and oversee junior nursing team members, ensuring high-quality care
Oversee infection control standards and cold chain compliance
Work within NMC Code of Conduct and practice standards
Manage recall systems for chronic diseases, screening, immunisations, and other areas
Implement management of chronic diseases and support evidence-based guidelines
Promote integrated services within Primary Care and multidisciplinary working
Ensure CQC compliance and maintain audit cycles
Job Responsibilities
Participate in annual appraisals and clinical meetings
Develop clinical protocols
Provide chronic disease management (Diabetes, COPD, Asthma, CHD/CVD)
Perform cervical screening, promote breast awareness, and administer injections and immunisations
Ensure safe storage and disposal of vaccines and drugs
Undertake venepuncture, electrocardiography, wound care, leg ulcer management, and dopplers
Advise on lifestyle changes and participate in smoking cessation and travel health services
Recognize signs of abuse and work with safeguarding teams
Apply infection control measures and manage emergencies
Provide family planning services and audit care outcomes
Maintain disease registers
Person Specification
Qualifications (Essential)
Nursing degree or relevant qualification with professional license for primary care nursing
Qualification in chronic disease management, cervical screening, venepuncture, electrocardiography, and wound care
Experience (Essential)
Experience in chronic disease management, cervical screening, vaccinations, wound care, infection control, and maintaining cold chain compliance
Desirable
Experience working in a GP practice, familiarity with System One, and independent prescribing qualification
Additional Requirements
This role is subject to a DBS check and current UK professional registration
