The Community Discharge Assessment Team aims to prevent avoidable hospital admissions and works closely with the multi-disciplinary team, providing expertise in discharge planning and ensuring a seamless transition between hospital and community services. CDAT Discharge Facilitators are a mixture of Registered Nurses, Physiotherapists, and Occupational Therapists. Our main areas of work are the Emergency Department (ED), Same Day Emergency Care (SDEC), and the acute assessment wards, collaborating with community services.
The team holistically assesses patients to identify opportunities for timely discharge, allowing patients to be safely managed in the community and avoid unnecessary hospital admissions.
They work with multidisciplinary teams to deliver optimal outcomes. Recently, the team established a new therapy offering to provide tailored OT support to facilitate discharge, especially in ED, SDEC, and acute wards, aiming to reduce length of stay and avoid admissions.
This therapy support enables Discharge Facilitators with backgrounds in Physiotherapy or Occupational Therapy to maintain their skills through assessments and home/environmental visits, ensuring safe and timely discharges.
Main duties of the job
Discharge Facilitators assess patient needs, collaborate with community colleagues, and coordinate discharges to achieve positive outcomes.
The service operates 7 days a week, 365 days a year, from 08:00 to 20:00 on weekdays and 08:00 to 18:00 on weekends and bank holidays.
Each morning, the team holds a huddle to allocate wards, ED, and SDEC. They download patient lists from PPM+ and Symphony Live to identify patients needing assessment.
Facilitators visit patients, coordinate discharges, and attend ward MDTs to discuss their caseloads.
CDAT assesses patients aged 16 and above, working closely with clinical teams, patients, and families for holistic assessments and documentation.
Referrals are produced to various community and social services to support individual outcomes.
