Leeds is a cosmopolitan, vibrant city which is located within a half hour drive to the beautiful Yorkshire Dales. We have been leaders in developing the strength-based approach to delivering social care and have helped others develop this positive way of working with people. We are currently recruiting a Wellbeing Worker for the Integrated Reviewing Team to support the delivery of Adult Social Care statutory duties, primarily with completion of initial and annual reviews.
As a Wellbeing Worker, you will be required to carry, manage, and monitor a variety of workload including diverse reviews with people in receipt of paid Adult Social Care services. You will need to use a person-centred approach within the parameters of agreed policies and practices, together with professional guidance and support from Social Workers, Senior Social Workers and Team Managers. You will need to bring Strength-Based approach to your work and work with individuals, families and carers to ensure the right support is provided to meet the assessed needs of the individual.
As a worker in the Integrated Reviewing Team you will need to demonstrate an ability to prioritise your work and be able to work quickly and efficiently to ensure you carry out the required amount of reviews. You will be required to be flexible and responsive in your approach to your caseload and the needs of the wider team and area as appropriate.
You will also need to demonstrate the ability to work in partnership with our co-located health colleagues to meet the needs and create a healthy and happy environment for the people of Leeds. This role will also involve signposting to other appropriate services, linking with the 3rd sector organisations as well as giving information and advice. You will also have the opportunity to work with professionals from other agencies and other teams across the council to support the effective delivery of services.

Job Description
Job Purpose:
Under the direction of a Team Manager and with support and direction from Social Worker colleagues, provide a high quality and comprehensive assessment and support planning service, advising adults with care and support needs. In doing so, promote the principles of wellbeing as described within the Care Act 2014.
Core Responsibilities:
- To work within a community or hospital based team, providing person centred support in line with the Care Act 2014.
- Contribute to the promotion of improved outcomes for individuals, families, carers and groups from diverse and disadvantaged communities.
- To undertake and contribute to the review of individual plans relating to the assessed needs of service users, their families and carers.
- Maintain and update case notes and other electronic records, write reports as required within the limits of data protection.
- With supervision, to carry a caseload of appropriately allocated cases and undertake assessments of service user needs using a person centred outcomes focused approach.
- Where required, prepare person centred support plans in conjunction with service user, their families and carers.
- Where safeguarding issues arise on the Wellbeing and Recovery Worker caseload, to escalate them to the manager and to assist the Social Worker to investigate and manage risks.
- To ensure that any concerns about practice and procedures are raised with the team manager.
- Organise work activities on their caseload and prioritise tasks as required.
- With support to undertake office duty as required.
- To provide general advice about the Council’s charging policy.
- Develop and maintain effective working relationships with other professionals within the directorate and partner agencies to ensure an integrated approach to the support planning and safeguarding of service users.
- Be responsible for understanding and applying all appropriate practice standards, policies and procedures.
- Responsible for assessing and working with the client to devise a support package that reflects the assessed needs of the service user, with support from a Social Worker where required.
- Be responsive to feedback and self-reflection around practise.
- Appropriately participate in meetings and relevant forums.
- Work closely with line manager using supervision and appraisal to develop own performance and skills through CPD.
- To use supervision and appraisal to continually develop professionally.
- To comply with the requirements of all Leeds City Council and Directorate policies and procedures and staff instructions, including responsibilities under Health and Safety policy and procedures.
- To actively promote Equality and Diversity throughout the service.
- To undertake any other duties and responsibilities appropriate to the grade and purpose of the job as may be determined by the directorate.
RESPONSIBILITIES:
- Undertake appropriately allocated non-complex assessments and support planning deemed suitable for allocation to Wellbeing and Recovery Worker. Self-directed support options should be at the heart of all support planning with service users.
- With supervision, develop and maintain effective relationships with service users, their families and carers: provide advice and support: help resolve conflict,
- With support and supervision, contribute to the enhanced screening of new referrals and people already in receipt of services.
- With supervision, work with individuals, families, carers and communities to help them make informed decisions enabling them to clarify and express their needs and contribute to service planning.
- With support from Social Workers and supervision from the Team Manager, to manage an appropriately allocated caseload of non-complex work, regularly monitor, review and evaluate changes in service user needs and ensure that all services contribute effectively to the support plan and remain an effective use of resources.
- Where safeguarding issues arise, to escalate them to the manager and work with the Social Worker to support, contributing to protecting the service users.
- Contribute to the development of the service by actively engaging in workshops, consultations and engagement events.
